Finding a Local Provider

Once you’ve determined eligibility or if you’re already receiving Medical Assistance benefits from Lycoming or Clinton County and need services, we help you find local community resources for mental health and/or substance use.

Please reference Community Care's website (link below) or contact Member Services. Community Care has an additional resource to help you and your family by:

  • Listening to your concerns related to mental health and substance use.

  • Providing educational support regarding the complaint and grievance process.

  • Helping you connect to community supports, available services, support groups, peer and family networks and other available county and public resources.

To help determine if you are eligible for services, Community Care, will utilize Medical Necessity Guidelines to see if you qualify to receive services.  In most cases, Community Care will ask your proposed agency to provide information as to why you need these services.  Community Care will review your request and make a clinical determination regarding your eligibility.
